Records Request Form Christ the Good Shepherd Parish retains the sacramental records of the following parishes in Oswego: St. Joseph, St. Paul, St Peter, St. Louis, St. Stephen the King and Sacred Heart (Scriba). Any person may request a sacramental certificate of his or her own sacramental record. In addition to the person who received the sacrament, the parents of a minor, one with legal guardianship of another, a pastoral minister at another Catholic parish, or a diocesan official may obtain an individuals sacramental information. If the person is deceased, the spouse or adult child may also request the record. Requests may be made in writing by mail or with this form. As these records contain sensitive information, only parish staff are allowed access to the register books. Please fill out the Records Request Form below and we will attempt to locate the record(s) as quickly as possible. Please note: Proof of identity may be required if the person is unknown to the pastor or parish staff. If you have any questions, please contact the parish office.
